When trying out for a sport at BLS, the following requirements must be completed prior to the first day of tryouts: 1. BLS Athletics Handbook: All athletes and their guardians must review the BLS Athletic Handbook and agree to its terms. 2. Sports Clearance: All candidates must be cleared by a BLS school nurse in order to participate in athletics even if you have played a sport in previous seasons. There is no longer a clearance form but you upload a physical on file through the registration form. Physicals are active within the last 13 months. 3. Concussion Course and Concussion Information: All players and parents must complete an online concussion course annually. The course can be found here or at www.cdc.gov/concussion. This should be done before filling out the online registration (see below). Players and parents must disclose any head injuries and/or concussions where indicated on the online registration form. 4. Opiate / Substance Abuse Education: All student-athletes and parents must read the information regarding opiate and substance abuse provided in this file labeled "Opiate Substance Information". Additional information is provided here as well. You will be required to acknowledge that you have read the information prior to registering. 5. MIAA DEI Pledge / NFHS Implicit Bias Course: In an effort to affirm our commitment to diversity, equity and inclusion, all student-athletes must take the NFHS Implicit Bias Course and acknowledge reading and understanding of the MIAA DEI Pledge: https://miaa.net/gen/miaa_generated_bin/documents/basic_module/DEI_Pledge.pdf. 6.
